2025-2026 Forms and Documents

We’re excited to introduce a new way to turn in your financial aid documents at CSUN! If you’re selected for verification, you can upload your documents easily from your phone, tablet, or computer using Student Forms. While you could already upload your verification documents on the CSUN portal, this new way makes submitting documents faster, easier, and more convenient for you!

Each year, when you apply for financial aid, you may be selected for verification. This is a required process by the U.S. Department of Education and/or the California Student Aid Commission to ensure the accuracy of your financial aid application.

With Student Forms, you’ll be able to:

  • Easily upload documents from any device (phone, tablet, or computer)
  • Track your progress and receive automated reminders for missing or incomplete tasks
  • Complete forms electronically on your own time

We’re excited for you to take advantage of this new, convenient way to manage your financial aid tasks!

  1.  Go to Student Forms.
  2. Click on the ‘Create Account’ link.
  3. Enter/confirm your student information.
  4. Click the ‘Register Account’ button.

Once your account is set up, you will be able to securely upload any required documents and complete financial aid forms. You’ll also receive automated reminders about outstanding tasks and next steps.

Please note: This is a one-time account creation. You will be asked basic information to validate your identity, allowing the use of an electronic signature for future forms.

Follow the step-by-step instructions.

  1. Click on the CSUN Student Forms link. You will be redirected to CSUN’s webpage to enter your CSUN credentials.  Screenshot of CSUN Portal login page.
     
  2. Next, on the “Register Account” page, you will be prompted to confirm/enter your personal information. DREAM Students, please enter your DREAM ID in the Social Security Number field. Screenshot of Student Forms account registration student information confirmation page.
     
  3. After, you will receive a confirmation email with a link to confirm your email address and activate your account. Screenshot of an email sent to a student that includes an email confirmation and account activation link.
     
  4. Once your account is activated, you will be redirected to the Needs Action page in Student Forms. Screenshot of Student Forms Needs Action list.

Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to your parent. This form must be completed every year and submitted in-person.

Federal Direct Parent PLUS Loan Request Form
Submit this form to apply for the federal direct parent loan for undergraduate students (PLUS). For information about this parent loan program, go to https://www.csun.edu/financialaid/federal-plus-loan (Do not submit this form until you have been awarded your financial aid).
