Forms & Documents

You can submit your financial aid documents at CSUN through Student Forms, a secure and convenient way to upload required materials. If you’re selected for verification, you may upload your documents from your phone, tablet, or computer using Student Forms. While document uploads are also available through the CSUN portal, Student Forms provides a streamlined and user-friendly option to make submitting your documents faster and easier.

Each year, when you apply for financial aid, you may be selected for verification. This is a required process by the U.S. Department of Education and/or the California Student Aid Commission to ensure the accuracy of your financial aid application.

With Student Forms:

Easily upload documents from any device (phone, tablet, or computer)
Track your progress and receive automated reminders for missing or incomplete tasks
Complete forms electronically on your own time

Student Forms provides a convenient way to manage your financial aid tasks and submit required documentation securely.

Go to Student Forms.
Click on the ‘Create Account’ link.
Enter/confirm your student information.
Click the ‘Register Account’ button.

Once your account is set up, you will be able to securely upload any required documents and complete financial aid forms. You’ll also receive automated reminders about outstanding tasks and next steps.

Please note: This is a one-time account creation. You will be asked basic information to validate your identity, allowing the use of an electronic signature for future forms.

Follow the step-by-step instructions.

Click on the CSUN Student Forms link. You will be redirected to CSUN’s webpage to enter your CSUN credentials.
Next, on the “Register Account” page, you will be prompted to confirm/enter your personal information. DREAM Students, please enter your DREAM ID in the Social Security Number field.
After, you will receive a confirmation email with a link to confirm your email address and activate your account.
Once your account is activated, you will be redirected to the Needs Action page in Student Forms.
