To make your first appointment

Your first appointment with a therapist at UCS is called an Initial Evaluation. Before your Initial Evaluation, you will be asked to complete some paperwork similar to the forms you would complete before seeing a doctor for the first time. These forms will provide us with information about your history, family, and current concerns, and are valuable in helping us understand your needs.
Initial Evaluations are offered every day of the week, at a variety of times to best accommodate your schedule. Appointments can be scheduled up to two weeks in advance.
When you meet with your counselor, some of the things they will discuss with you include what led you to seek counseling services and any symptoms you may be experiencing. The counselor will share with you their recommendations and discuss options to address your concerns.

First Appointment
Once you have scheduled your Initial Evaluation appointment, we ask that you complete the Initial Evaluation Questionnaire and Consent Forms (UCS Informed Consent for Counseling Services) online prior to your appointment time. It usually takes 20-40 minutes. Please follow the instructions listed on our Forms page and don't hesitate to call us if you need assistance with these tasks at (818) 677-2366, option 1.
- Complete forms (visit our Forms page for instructions and MyHealth@CSUN portal information)
- Please call our support staff at (818) 677-2366, Option 1, 15 to 20 minutes before your Initial Evaluation appointment to do a “pre-check in” for your appointment.
- During the pre-check in, the support staff will confirm that you have completed all necessary paperwork
- Appointment questionnaire
- Informed Consent
- Notice of Privacy Practices (HIPAA Form)
- Support staff will provide you with instructions on how to access and complete the CCAPS (Counseling Center Assessment of Psychological Symptoms).
- Once you complete the CCAPS, your therapist will be notified to review your responses.
- The therapist will then call you by phone from one of the following:
- A CSUN campus line, identified with the prefix (818) 677-
- A blocked number if the therapist is not able to use their campus phone line
