Cayenne Wellness Center and Children’s Foundation (CWC) is a 501(c)3 organization dedicated to improving health outcomes and decreasing mortality related to Sickle Cell Disease (SCD). Our mission is to increase the quality of life for individuals diagnosed with SCD in California by ensuring expert, unbiased, and comprehensive care. POSITION SUMMARY:

The Sickle Cell Social Worker will play an important role on our team. The Sickle Cell Social Worker has expertise in understanding the impact of sickle cell disease on patients and their families and ensures the delivery of high-quality services to individuals living with Sickle Cell Disease and their families. Your work will include psychosocial assessments, supportive counseling, brief interventions for behavioral health, resource coordination and navigation, and advocacy support. There are additional opportunities for program development, research, and participation in a wide variety of activities. As an inherited disease, the patients have complex psychosocial needs which stem from chronic medical conditions across their lifespan. In the United States, it is estimated that Sickle Cell Disease occurs among about 1 out of every 365 Black or African-American births and among about 1 out of every 16,300 Hispanic- American births. Many patients experience depression or anxiety living with sickle cell disease.

The clinician will serve as a member of the multidisciplinary team and collaborate to ensure the behavioral health needs of the patients and families are met. Clinical responsibilities for services include but are not limited to, conducting psychosocial assessments, individual therapy, crisis interventions, leading groups and development of programs for parents, formulating psychosocial diagnoses and treatment plans, and implementing effective, evidence-based treatments for patients and family members. The clinician will also provide education/teaching regarding behavioral health conditions and treatment to providers and staff. As an integral part of the multidisciplinary team, the sickle cell social worker will complete psychosocial assessments, provide supportive counseling, and work with families to address psychosocial needs. The sickle cell social worker will have the opportunity to build relationships with families over time and to work with families impacted by both acute and chronic illness. Primary focus: caregivers and their children.

CORE DUTIES:

  1. Attendance at CWCs weekly multidisciplinary team meetings and education program offerings;
  2. Attend continuing education training necessary to work toward licensure;
  3. Establish and maintain effective working relations with patients/clients;
  4. Evaluate case problems and provide leadership and consultation in the development and implementation of case plans;
  5. Understand, interpret, and apply regulations and departmental policies and procedures;
  6. Plan, schedule, and conduct individual and group meetings;
  7. Provide clinical case management and therapeutic services.
  8. Interviews patient/patient's social support system to complete and initiate psychosocial evaluation;
  9. Provides psychoeducational groups to support patients and the patient's social support system.
  10. Provides ongoing assistance to patients/patients’ social support systems in coping with stress related to hospitalization, disability, and chronic/terminal illness.
  11. Works collaboratively with the patients and their social support system to establish and implement a plan of care with a multidisciplinary team that meets the patient's identified psychosocial needs.
  12. Conceptualize, develop, and implement a comprehensive system of care program to support patients and the patient's social support system (a system of care ranging from medical, psychosocial, dental, educational, legal, housing, food, spiritual, and financial stability).
  13. Engage with community stakeholders and agencies to build capacity and collaboration.
  14. Develop and conceptualize evaluation feedback to determine the effectiveness of services
  15. Conduct a 3-5 year strategic plan.
  16. Conduct outreach to network with local, state, and national organizations that are providing services for patients and patients’ support systems.

RESPONSIBILITIES, SKILLS, AND ABILITIES:

  • Knowledge of ethical standards, principles, and practices of social work counseling, and human behavior.

  • Knowledge of the principles, methods, and practices of effective supervision.
  • Knowledge of the theories of group interaction and therapeutic intervention.
  • Functions as a member of the interdisciplinary team and provides Clinical
  • Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health (Continuously).
  • Ability to provide leadership in the development and administration of social work programs.
  • Ability to demonstrate the concepts of ethical social work practice.
  • Adhere to the NASW Code of Ethics.
  • Ability to maintain effective relationships with staff, other disciplines, and community agencies and to interpret programs to the public.
  • Ability to participate in recruitment, selection, and orientation of professional social work staff.
  • Ability to write meaningful, concise, and accurate reports and correspondence.

QUALIFICATIONS:

  • Education: Requires Master's Degree from an accredited school of SocialWork (MSW) or higher.
  • Requires a level of knowledge of the Social Work practice at the macro level.
  • Requires knowledge of community resources and agencies in relation to health, illness, disability, and social determinants of health at the local, state, and national levels.
  • Demonstrates cultural humility and comfort with a diverse patient population.
  • Cultural humility is essential.
  • Bicultural/Bilingual competency is a plus.
  • Ability to work in an interdisciplinary team.
  • Computer literacy with the ability to use standard word processing,spreadsheet programs, e-mail (i.e. MS Word, Excel, Outlook), and webbrowsers.
  • Ability to multitask in a dynamic work environment.

The Clinician (MSW/MFT/PCC) is working as part of a multidisciplinary team servicing metro lines and stations and nearby encampments in the LA metro area to address the unhoused population.  Clinicians will work on a team with outreach workers, medical staff, SUD counselors, and case managers in order to increase access to housing and services for unhoused riders. This program is in coordination with the LA County Department of Health Services (DHS). The right candidate should have a passion for helping the unhoused and want to work in a team environment.

Protective Services Worker – Public Conservator, Department of Disability and Aging Services Division

The Office of Conservatorship Services has an average caseload of 640, with approximately half of these individuals placed outside of San Francisco County in either locked psychiatric facilities or residential care homes. Therefore, travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350.

 

The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders. Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).

Protective Services Worker – Adult Protective Services, Department of Disability and Aging Services Division 

Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), Human Services Agency, and the Department of Disability and Aging Services (DAS). Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse. The PSW performs professional investigations and case management services to victims of abuse. This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an “elder” or “dependent adult;” assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed.

Case Manager

The Case Manager provides comprehensive case management support services to Pepperdine University, with a particular focus on assisting individuals served by the Student Title IX Coordinator and the Student Care Team. As a key member of the Student Affairs staff, this individual works closely with students, offering guidance and support while serving as a Christian role model. By ensuring that students receive the necessary resources and support, this role helps empower them to thrive, aligning with the University’s mission to prepare students for lives of purpose, service, and leadership.

Duties

  • Participate in Student Care Team (SCT), Title IX meetings, and other collaborative efforts.
  • Interface with faculty, staff, and parents regarding students needing treatment plans, accommodations, or leaves.
  • Document case management, maintain data, and provide reports regarding students receiving case management services.
  • Intake-manage and assess incoming calls and referrals to SCT and Title IX.
  • Facilitate Seaver 200 Course for first year students on a weekly basis (for seven weeks per semester) as needed, serve on the Residential Emergency Response Team.
  • Provide case management services for students served by the Title IX Coordinator and the Student Care Team; will serve as a resource to other campus departments as needed; engage in research to inform case management best practices; participate in Student Affairs initiatives.
  • Manage crisis situations and provide crisis intervention. Travel as needed to support students in crisis.
  • Identify and develop relationships and referral procedures with off-campus providers.
  • Support programming and prevention education efforts for Student Care and Title IX.
  • Perform other duties as assigned.

Skills and Qualifications

Ability to work well with college students, including those in distress. Capacity to manage under high pressured situations. Demonstrated ability to maintain confidentiality and professionalism. General office software experience.

Preferred: Licensed social worker or other mental health professional. 3 years of case management experience in hospital setting or higher education. Demonstrated case management skills (assessment, coordinated care planning, service linkage, program development, outreach and training facilitation skills). Ability to recognize and maintain healthy professional boundaries among students and colleagues. Microsoft Office, Database maintenance, Excel, PowerPoint, Canva, and ability to navigate social media platforms.
