Student Travel
Group Travel
Faculty/Group Leader to complete the following:
- Approval of Travel Request (PDF)
- Conference Agenda and Travel Itinerary
- Academic Field Trip Participant List (PDF)
- Travel Advance Request, if any (PDF)
Students must complete the following:
Additional Forms if Driving (even to the airport) and/or Flying
- Student Authorization to Operate Privately Owned Vehicle (PDF)
- Copy of valid car Insurance
- Copy of valid driver's license
- Student Air Travel Voluntary Participation (PDF)
Please note the Department should keep the original Additional forms for students and provide the Dean's office with a copy in case of emergency.
Students Traveling Alone
Student must provide the following:
- Approval of Travel Request (PDF)
- Conference Agenda and Travel Itinerary
- Documentation why the student is traveling alone (Ex. Is it required for class, internship, etc.)
