Are you looking for an exciting new challenge? Valley Care Community Consortium is seeking a passionate, driven, and dynamic Community Navigator to join our team! As a Community Navigator, you will: engage with community partners to develop and implement a referral pathway for residents of the San Fernando Valley, help map community assets while collaborating with local stakeholders, and, working alongside a team of talented professionals in a collaborative environment, create linkages and referrals to health (medical and behavioral), social and legal services, and other American Rescue Plan Act- funded programs (ARPA).
