Authorization to Release Information
Submit this form to authorize the Financial Aid & Scholarship Department to release student or parent information to your parent. This form must be completed every year and submitted in-person.

2026-2027 Status Change Notification Form
Submit this form to report enrollment status changes for the fall and/or spring term. You can access this form by going to the CSUN home page and logging in to the CSUN Portal. Once you are logged in, click the My Finances menu and select Financial Aid & Scholarships. Scroll down to the Financial Aid Essentials section and click Financial Aid Forms. It may take up to 4 weeks for this request to be processed.

Loan request forms for the 2026-2027 academic year are not yet available. Students cannot submit a loan request at this time. Please check back later for updates.
